Need Some Help Filling out the Registration Form? Use our helpful tips!

• SCIDES ID - Returning student: Only fill out if you have taken previous courses at SCIDES.
  
  
• Student info boxes: Fill out as detailed as you can.
  
  
• Grade: Select the appropriate box
  
  
• Personal Education number: There are 9 digits in a PEN. Students need to insert their PEN (Personal Education Number) assigned by the Ministry of Education here. They can obtain the number from their home school or from transcripts, report cards etc. It is a nine-digit number. Usually it can also be obtained by contacting the last BC school the student was enrolled in. If a student cannot locate their PEN or never was part of the BC education system simply put 999999999 in this box so that it can go through when submitted.
  
  
• Courses Box: Here simply select one of the Guitar or Piano courses in the list. The grade level doesn’t matter unless you are actually in that grade taking a course for credits. You will be able to select your actual level once you have registered.
  
  
• Proof of BC residency box: A copy of one document needs to be sent to the school. This can be a copy of your birth certificate, citizenship card, Permanent Residency Card, Care Card, Landed Immigrant papers, Canadian Passport, or study/work permit/visa.
  
  
  • This document can be scanned and attached to this registration form.
  • It can also be faxed to 250-378-1447
  • If the document is sent after submitting the registration form send it to: tswetlis@scides.ca.
  • The document can also be mailed to:

• Once you register you will be contacted and also sent the passwords etc. to the ‘Classroom’ that you have now registered in.
  
  
• After you have submitted the application form, above, an "Activation Assignment" must be completed within 10 days of submitting the application form.  This satisfies the BC Ministry of Education's mandate for Distributed Learning.
